Ticket Order Forms

Below you can download the forms necessary to apply for the Sun Metro IDs for reduced fare. Please mail the completed application(s) to the mailing address to the left or bring them to the Bert Williams Downtown Santa Fe Transfer Center. In order to receive a Sun Metro ID you must come to the Bert Williams Downtown Santa Fe Transfer Center to have your picture taken. Groups of 20 or more may contact us to have our mobile ID representatives come to your location.

	(SM B) Application for Reduced Fare ID This form is for Senior Citizens, Medicare recipients and Students. For qualifications and more information, please see Applying for Reduced Fare.
	(SM A) Application for Disabled ID This form is for Disabled applicants (including Disabled Students). For qualifications and more information, please see Applying for Reduced Fare.
	Sun Metro LIFT Application This form is for LIFT applicants. For qualifications and more information, please see Applying for Reduced Fare. Note: Please mail the Lift Application to: Sun Metro LIFT 5081 Fred Wilson El Paso, TX 79906
